Knowledge Poisoning Attacks on Medical Multi-Modal Retrieval-Augmented Generation

Paper · 2026 · Peiru Yang, Haoran Zheng, Tong Ju et al.

Summary

Medical multimodal RAG systems pair images such as X-rays with clinical text in a retrieval database, and injected adversarial entries can compromise them. Most earlier poisoning attacks assume the attacker already knows future user queries, which rarely holds in real deployments. The authors present M3Att, an attack that needs only rough knowledge of the knowledge-base distribution, which could be estimated through black-box use of the system. The attack targets both stages of the pipeline:

  • Retrieval: it clusters image embeddings from a reference pool and treats the cluster centres as proxy targets. It then applies imperceptible PGD perturbations to poisoned images so they surface for many unseen queries.
  • Generation: it rewrites the paired text so it exploits genuine diagnostic uncertainty. As a result, LVLMs with strong medical priors are less likely to notice and correct the misinformation.

Across five LVLMs, five datasets and four medical tasks, M3Att outperforms baseline attacks, and common corpus-filtering defences barely weaken it. The work exposes a realistic, query-agnostic threat to clinical AI and supports red-teaming of such systems.

Key points

  • Threat model: the attacker has a limited injection budget and cannot see the model's parameters, the queries users submit, or the contexts the system retrieves. The retriever ranks the top-k image-text pairs by image-to-image similarity.
  • Text poisoning: a controlled LLM editor applies fine-grained severity shifting, prior-constrained diagnosis distortion and risk association corruption.
  • Case study: after PGD optimization, a poisoned image's Top-5 hit rate rose from about 0.01% to about 5% of test queries.
  • Ablation: removing either retrieval hijacking or misinformation injection noticeably restores downstream utility, so the two modules depend on each other.
  • Defences:
    • Pre-retrieval filters (Image Clustering, Text Clustering, Image-Text Consistency) leave the retrieval attack success rate almost unchanged.
    • Stronger retrieval-time defences on CLIP-based retrievers (perplexity filtering, anomaly detection, score-based pruning) also fail to stop the attack.
  • Scope and code: evaluation covers only 2D modalities (chest X-rays, histopathology patches), not 3D or video data. Code is at github.com/ypr17/M3Att.
