CMB: A Comprehensive Medical Benchmark in Chinese
Paper · 2023 · Xidong Wang, Guiming Hardy Chen, Dingjie Song et al.
Summary
Evaluating LLMs for medicine in China is hard because translated English benchmarks miss local practice, especially traditional Chinese medicine (TCM). The authors therefore built CMB, a medical benchmark created natively in Chinese. It has two parts:
- CMB-Exam is a large set of multiple-choice items drawn from qualification and coursework exams. It covers physicians, nurses, technicians and pharmacists, plus undergraduate and graduate-entrance tests.
- CMB-Clin contains expert-curated clinical case records. Models must analyse each case over several rounds of questions.
The authors tested general models (ChatGPT, GPT-4), Chinese chat models and medical-domain models.
GPT-4 and some recent open models score above the 60% pass mark used for licensing. However, accuracy varies widely by professional level and between TCM and Western medicine. Strong exam scores often fail to carry over to case-based dialogue. This suggests some models are tuned for multiple-choice formats rather than real consultation. Automatic scoring with GPT-4 closely matches expert judgments, which makes open-ended evaluation cheaper. CMB gives a culturally grounded tool for tracking medical LLM progress.
Key points
- CMB-Exam size and structure: 280,839 questions in 6 categories, 28 subcategories and 174 subjects. Each question has 4–6 options, and more than one answer can be correct.
- CMB-Exam splits:
- test: 11,200 items (400 per subcategory)
- dev: 280 items, with explanations
- train: 269,359 items
- Data sources: mostly Medtiku, used with permission. OCR errors were corrected manually.
- CMB-Clin: 74 real, complex case records taken from diagnostic teaching materials. Responses are rated on Fluency, Relevance, Completeness and Proficiency. Completeness was the hardest dimension.
- Main results:
- GPT-4, Qwen-72B-Chat and Yi-34B-Chat exceed 60% accuracy on CMB-Exam.
- HuatuoGPT-II-34B beats GPT-4 on CMB-Exam but trails it, and its base model Yi-34B, on CMB-Clin.
- GPT-4-based and expert rankings agree strongly (Spearman 0.93).
