Tailored Emotional LLM-Supporter: Enhancing Cultural Sensitivity

Paper · 2026 · Chen Cecilia Liu, Hiba Arnaout, Nils Kovačić et al.

Summary

LLMs are increasingly used for emotional support, but it is unclear whether their replies respect the cultural context of a person in distress. Advice such as cutting family ties or simply seeing a therapist can feel alienating where family duty or mental-health stigma matter. Earlier work was limited to a few hand-crafted Pakistani scenarios. The authors built CultureCare, a dataset of real Reddit post–response pairs from four cultures: Arabic, Chinese, German, and Jewish. In-culture annotators labeled spans for distress and its intensity, cultural signals and their categories, support strategies, and empathy, and a second group of reviewers verified the labels. Using this resource, they compared four prompting-based adaptation strategies across three state-of-the-art LLMs. Evaluation combined LLM-as-a-Judge, in-culture human raters, and clinical psychologists. Adapted models beat anonymous peer replies, but merely asking a model to role-play a culture was not enough. Detailed guidelines plus explicit, contextualized cultural signals worked best. The work offers a first benchmark for culturally sensitive support and points to uses in training therapists.

Key points

  • CultureCare comprises 462 posts carrying 4,293 annotations overall. Of these, 1729 are distress messages, 1041 are support strategies, and 1523 are cultural signals. Texts are in English and in the cultures' native languages, since culture is not defined by language.
  • Data were collected by searching mental-health subreddits with culture keywords and the reverse (e.g., r/china with "depression"). Rule-based and LLM-based filters then removed noise. Ethics approval: TU Darmstadt EK 121/2024.
  • Psychologists judged that LLM replies did not add or escalate distress in 80% of cases, compared with 28% for Reddit replies. They rated 88% of LLM replies as useful for cross-cultural therapy training: 46% as strong examples and 42% as needing minor edits.
  • Human–judge agreement on cultural awareness was moderate (about 0.6 for Arabic and 0.47 for Chinese). Humans and the judge mostly agreed on the best strategy per model, but not on the weaker ones.
  • The study covers single-turn interactions only. Code and data are at github.com/UKPLab/eacl2026-culturecare.
