RA-RRG: Multimodal Retrieval-Augmented Radiology Report Generation with Key Phrase Extraction

Paper · 2026 · Jonggwon Park, Byungmu Yoon, Soobum Kim et al.

Summary

Multimodal LLMs can draft chest X-ray (CXR) reports, but they are costly to train, need large datasets, and often hallucinate. A common hallucination is a reference to earlier examinations when only one image is available. RA-RRG addresses this with retrieval instead of LLM fine-tuning. First, RadGraph structures are built from the FINDINGS sections of reports. An LLM then refines them into short, clinically meaningful key phrases and drops comparisons with prior studies and other content that the image cannot support. Next, a multimodal retriever learns to match each image with its phrases. It follows the TranSQ/DETR design and fuses features from several vision encoders. At inference, the retrieved phrases condition an off-the-shelf LLM, which writes the report. On MIMIC-CXR and IU X-ray, the framework reaches state-of-the-art CheXbert scores and competitive RadGraph F1, at a small fraction of the compute that MLLMs need. It also handles multi-view inputs without extra training by pooling phrases retrieved per image. This makes it a practical and lower-hallucination option for clinical settings.

Key points

  • Low training cost. Training takes about 18 GPU-hours, compared with more than 200 GPU-hours for comparable MLLMs, and no LLM fine-tuning is needed.
  • Retriever design. Visual tokens from encoders such as CLIP and DINOv2 are interpolated and concatenated along the channel dimension. A frozen text encoder embeds the phrases, with NEFTune-style uniform noise and L2 normalization added during training.
  • Evaluation results. On MIMIC-CXR and IU X-ray, RA-RRG achieves state-of-the-art CheXbert metrics and competitive RadGraph F1. Its NLG scores are lower because the LLM rephrases the retrieved findings.
  • Multi-view and hallucinations. Combining frontal and lateral phrases recovers findings that the frontal view alone misses, such as pleural effusion. The method suppresses both comparative and object-level hallucinations.
  • Limitations. The phrase database is built only from training-set phrases. Retrieval errors cannot be corrected during generation, and the work has no evaluation by radiologists. Code is available at github.com/deepnoid-ai/RA-RRG.
