LLMs Can Simulate Standardized Patients via Agent Coevolution

Paper · 2025 · Zhuoyun Du, Lujie Zheng, Renjun Hu et al.

Summary

Medical training relies on standardized patients (SPs), actors who portray real cases. They are costly to prepare, and the role can strain their well-being. LLM-based SPs exist, but earlier work mostly improved retrieval or relied on expert feedback. The harder problem is teaching a knowledgeable model to answer like a lay patient who holds back information, with little human supervision. The authors propose EvoPatient, a multi-agent framework without weight updates. It splits a diagnosis into phases: complaint generation, triage, interrogation and conclusion. Doctor agents question a patient agent built from real, de-identified hospital records, and they can recruit specialists from other disciplines to vary the questions. A coevolution mechanism checks good dialogues and saves them in dynamic libraries. The patient agent draws few-shot examples and refined requirements from these libraries, while doctor agents learn shortcuts for sharper questions. Given only general SP requirements, EvoPatient beats existing reasoning methods by over 10% in requirement alignment, and human doctors prefer its answers. This points to cheaper, scalable virtual SPs for clinical education.

Key points

  • The patient agent uses 5,000 synthetic profiles that vary family, education, economic status and Big Five personality traits. RAG pulls the relevant facts from each medical record.
  • The best balance of cost and quality came after evolving on about 200 cases over roughly 10 hours, and the gains generalized well.
  • Clinicians with a graduate medical degree, or working toward one, each judged 500 pairs of responses (EvoPatient vs. a baseline) for preference.
  • Using the medical model Spark-Pro as the doctor agent gave only small gains, because its questions were fairly fixed. Building doctor agents from scratch therefore mattered.
  • Stated limitations: the patient agent can mishandle unfamiliar or trick questions, doctor agents generalize poorly to very different diseases, and automatic dialogue evaluation remains hard. Code: github.com/ZJUMAI/EvoPatient.
